This role will work closely with hospitals, managed care teams, physicians, discharge planners, and healthcare providers to identify patients, coordinate care plans, and facilitate smooth transitions from acute care settings to home-based infusion services. The ideal candidate is a Registered Nurse with strong clinical knowledge, relationship-building skills, and an interest in business development or healthcare sales.<br> 

<br><strong>Responsibilities as the Clinical Outreach RN:</strong><br>
