Job Overview - Wellness Coach (Case Manager):
Are you passionate about empowering individuals with mental illness to thrive in their community? We're seeking a dedicated Wellness Coach for our Brooklyn office. In this role, you'll engage with residents, teach essential life skills, and connect them to critical services that support their recovery and independence.
Location: Brooklyn, NY
Compensation: $23/hour
Schedule: 9:00 AM - 5:00 PM, Tuesday to Saturday
Responsibilities as the Wellness Coach:
Engage individuals with mental illness, teaching community living and housing maintenance skills.
Conduct group meetings focused on wellness management.
Complete service plan reviews for residents.
Assess, correct, and report on apartment and personal living space conditions.
Provide case management, collaborate on rehabilitation goals, and develop housing support plans.
Qualifications for the Wellness Coach:
Bilingual (English/Spanish) preferred.
Bachelor’s Degree in human services or related field.
Minimum of two years’ experience working with individuals with mental illness.
Proficient in word processing with excellent writing skills for developing treatment plans.
