Job Overview – Wellness Case Manager:
Compensation: $25/hour
Location: Brooklyn, NY
Schedule: Tuesday to Saturday, 9:00 AM to 5:00 PM
TAG MedStaffing is hiring a Wellness Case Manager in Brooklyn, NY for our client. In this role, you’ll support individuals with mental illness in achieving recovery, maintaining housing stability, and building life skills for independence. You’ll lead wellness groups, develop service plans, and connect residents with community resources. Ideal for compassionate professionals committed to promoting empowerment and long-term well-being.
Responsibilities as the Wellness Case Manager:
Resident Support: Help residents build life skills that promote independence, stability, and community integration.
Wellness Programs: Facilitate group sessions on wellness management, recovery, and self-determination.
Service Planning: Create and update individualized housing and rehabilitation plans based on resident goals.
Monitoring & Documentation: Conduct apartment inspections, ensure safe living conditions, and maintain progress records.
Case Coordination: Collaborate with residents, clinicians, and community providers to connect individuals with needed services.
Qualifications for the Wellness Case Manager:
Education: Bachelor’s degree in Human Services, Social Work, Psychology, or a related field required.
Experience: 2+ years working with individuals living with mental illness or in a supportive housing environment.
Technical Skills: Proficient in Microsoft Word and other documentation tools, with strong writing skills for treatment and service planning.
Industry Knowledge: Understanding of community-based mental health services, housing programs, and recovery-focused practices.
Skills & Attributes: Compassionate and organized professional with strong communication and interpersonal abilities (bilingual in English and Spanish preferred).
