Job Overview - Social Worker:
We are seeking a bilingual Social Worker (English/Spanish or English/Chinese) for a temporary assignment supporting case management services, with travel to multiple sites.
Location: New York, NY
Compensation: $55-$58/hour
Hours: 9:00 AM to 5:00 PM, Monday to Friday
Responsibilities as the Social Worker:
Conduct assessments using PHQ-9, MSSI-SA, and GAD-7.
Refer clients to mental health professionals within five business days.
Coordinate and facilitate monthly group counseling and education sessions.
Follow up on mental health and substance use treatment plans monthly.
Document assessments, referrals, and interactions in the database.
Provide case management and assist with care plans to promote healthy aging.
Prepare required documentation and cover for the Clinical Manager when needed.
Handle more involved cases as assigned.
Qualifications for the Social Worker:
LMSW Degree with a minimum of two years of experience.
Bilingual in English/Spanish or English/Chinese.
Experience with the geriatric population, mental illness, and substance use.
Proficient in assessments and care coordination.
Strong computer skills, especially in Microsoft Office Suite (Word, Outlook, Excel).
