Job Overview – Social Worker (MSW):
Advance your healthcare career by joining our client as a Social Worker! This temporary role involves conducting psychiatric and other assessments, developing and implementing person-centered treatment plans, and providing supportive services through evidence-based practices.
Compensation: $28/hour
Location: Bronx, NY
Schedule: 9:00 AM to 5:00 PM, Tuesday to Saturday
Responsibilities as the Social Worker:
Client Assessment: Assess clients’ substance abuse, health, mental health, and other care needs; recommend internal and external referrals.
Counseling: Provide individual and group counseling; prepare psycho-social evaluations.
Psycho-Education: Facilitate groups on relapse prevention, health literacy, and navigating mental health and substance abuse systems for residents and their support networks.
Case Management: Provide comprehensive case management services by assisting clients in improving self-care, interpersonal, and vocational skills.
Housing Stability: Assist clients in achieving and maintaining housing stability.
Discharge Planning: Manage discharge planning and provide support to clients for 90 days post-discharge.
Documentation: Manage client documentation and provide program narrative and statistical reports.
Qualifications for the Social Worker:
Education: Master’s in Social Work (MSW) required; New York State Licensed Master in Social Work preferred.
Experience: 2+ years of experience with homeless populations and persons with chronic health, mental health conditions, and/or substance abuse disorders.
Skills: Bilingual/Spanish skills a plus; ability to learn and provide evidence-based treatment practices.
Knowledge: Familiarity with treatment interventions and wellness management practices for chronic health conditions, serious mental illness, and substance abuse disorders.
Technical Proficiency: Ability to administer assessment tools, deliver counseling services, and efficiently record services provided, including in systems like AWARDS or CARES.
