Job Description

Job Overview – Social Worker (LMSW):
Join our client's expanding healthcare team as a Social Worker (LMSW) in New York City! This role involves providing on-site clinical assessment and support for outreach teams, determining homelessness history, services needed, and appropriate placement options. The Social Worker utilizes Harm Reduction and Housing First strategies to ensure high-quality care and advocacy for clients being discharged.

Location: New York, NY
Compensation: $60,000 - $62,000/year
Schedule: 9:00 AM to 5:00 PM, Monday to Friday


Responsibilities as the Social Worker:
  • Strategy Promotion: Implement Housing First, Harm Reduction, CPI, Motivational Interviewing, and trauma-informed care models.
  • Assessments: Complete 9.58 certification and handle all assessments and removals for the shift.
  • Community Liaison: Collaborate with community partners and follow up on homelessness conditions and community concerns.
  • Patient Advocacy: Partner with hospital staff to advocate for patients being discharged.
  • Relationship Building: Develop strong relationships with external partners, NYPD, EMS, and other stakeholders.
  • Team Support: Guide outreach teams in managing difficult situations and de-escalating conflicts.
  • Documentation: Ensure accurate and timely reporting and documentation.
  • Case Management: Manage a caseload of clients and attend meetings with community partners.
Qualifications for the Social Worker:
  • Education: NYS Licensed Masters Social Worker (LMSW) required, or within the first six months of employment.
  • Experience: Previous clinical, supervisory, and administrative experience preferred.
  • Skills: Understanding of Housing First, Motivational Interviewing, Harm Reduction, Health Systems, and Crisis Intervention experience.
  • Technical: Proficiency in Microsoft Office Suite.
  • Attributes: Detail-oriented, collaborative, and able to manage team tasks.
  • License: Valid NY or NJ driver’s license.
  • Language: Bilingual preferred.
