Job Overview - Social Worker (Behavioral Health):
Take on the full-time role of Social Worker specializing in Behavioral Health with our client in Elko County, NV. You’ll provide personalized care to patients, collaborating with healthcare teams to create comprehensive treatment plans and support discharge planning.
Compensation: $74,000 - $87,000/year
Location: Elko County, NV
Schedule: Monday to Friday, 8:00 AM to 5:00 PM
Responsibilities as the Social Worker (Behavioral Health):
Patient Care: Provide direct and individualized care to patients by conducting interviews and assessments.
Coordination: Arrange and prioritize necessary internal and external interventions and resources for patients.
Case Management: Participate in case reviews and collaborate with healthcare teams to ensure adherence to treatment plans.
Discharge Planning: Assist with discharge processes, including identifying and coordinating resources such as transportation, housing, and social services.
Screenings: Perform Medicaid screenings as required and assist patients with Safety Net applications.
Qualifications for the Social Worker (Behavioral Health):
Education: Master’s degree in Social Work required.
Experience: 1+ years of experience in a Behavioral Health or Social Work setting.
Licensure: NV Licensed Clinical Social Worker (LCSW) license required.
Certifications: Basic Life Support (BLS) certification required.
