Job Description

Role Overview:
Our client, a transitional foster facility for vulnerable adolescents in Brooklyn, is seeking a Social Services Coordinator to assess treatment needs, coordinate care services and provide direct interventions uplifting this community. Embodying a “See What Can Be” spirit, the ideal candidate will work alongside specialists conducting evaluations, facilitating youth transition, enabling family reunification, and guiding society’s most impressionable towards productive futures.

Compensation: $35.00 - $38.00 / HR
Location: Brooklyn, New York
Schedule: Monday – Friday, 9 am – 5 pm; 35 hours/week

•    Performing intake psychosocial evaluations
•    Developing integrated treatment care plans
•    Coordinating discharge transitions and placement
•    Providing individual and group counseling
•    Interviewing family members to inform care
•    Collaborating across medical disciplines
•    Responding sensitively to behaviors and crises
•    Promoting trauma-informed support frameworks
•    Maintaining timely documentation

•    Master’s degree in Social Work (MSW)
•    2+ years experience with foster youth
•    Proven clinical evaluation and care coordination talents
•    Strong track record cultivating community relationships
•    Excellent crisis intervention abilities
•    Bilingual in English/Spanish ideal
