Role Overview:
A human services organization in the Bronx is seeking an experienced Senior Social Worker to evaluate clients and provide integrated healthcare and psychosocial interventions. Leveraging a passion for uplifting vulnerable groups, the ideal candidate will conduct assessments, deliver counseling, facilitate groups focused on self-management skills, coordinate discharge transitions, and promote recovery through evidence-based modalities tailored to individual needs.
Compensation: $30.00 / HR
Location: Bronx, New York
Schedule: Monday - Friday from 9 am to 5 pm
Responsibilities:
• Assessing substance use issues and mental health needs
• Developing customized treatment plans
• Providing group and individual counseling
• Assisting clients with healthcare system navigation
• Supporting housing transitions and stability
• Managing documentation and reporting
• Enabling continuity of care through referrals
Qualifications:
• Master's in Social Work (MSW)
• Licensed in New York
• 2+ years experience with relevant populations
• Knowledge of therapeutic best practices
• Strong engagement abilities
• Bilingual in English/Spanish is a plus
