Role Overview:
A Bronx-based healthcare organization is seeking an experienced Licensed/Credentialed Senior Social Worker to assess clients and coordinate evidence-based psychosocial care interventions. Leveraging a passion for guiding vulnerable groups, the ideal candidate will perform evaluations, provide counseling focused on relapse prevention and independent living skills, facilitate discharge transitions, enable care navigation, and promote recovery through individualized treatment planning.
Compensation: $40.00 - $42.00 / HR Location: Bronx, New York Schedule: Monday - Friday from 9 am to 5 pm
Responsibilities:
• Assessing substance use issues and mental health needs
• Developing customized psychosocial treatment plans
• Delivering group and individual counseling
• Training clients on healthcare self-advocacy
• Assisting housing transitions and stability
• Managing documentation and reporting
• Coordinating external specialty referrals
Qualifications:
• MSW required; Licensed preferred
• 2+ years experience with relevant populations
• Excellent understanding of local resources
• Strong engagement and communication talents
