Job Overview – Senior Medical Social Worker (LMSW/LCSW):
Join our client as a Senior Medical Social Worker in New York, NY. In this role, you will provide comprehensive assessments, develop person-centered treatment plans, and deliver evidence-based services to help clients achieve stability and independence. You will collaborate with the Senior Program Director to manage referrals, screenings, and intakes, ensuring clients receive the care and support they need to thrive.

Compensation: $40 - $42/hour
Location: New York, NY
Schedule: Tuesday to Saturday, 9:00 AM to 5:00 PM


Responsibilities as the Senior Medical Social Worker:
  • Client Assessment: Evaluate clients’ substance abuse, health, and mental health needs; recommend and facilitate appropriate internal and external referrals.
  • Counseling Services: Provide individual, group, and family counseling to clients and their support networks.
  • Educational Support: Facilitate psycho-education groups on relapse prevention, health literacy, and navigating mental health and substance abuse systems.
  • Case Management: Deliver comprehensive case management services, assisting clients in improving self-care, interpersonal, and vocational skills.
  • Housing Stability: Support clients in achieving and maintaining stable housing arrangements.
  • Discharge Planning: Oversee discharge processes and provide ongoing engagement and support for up to 90 days post-discharge.
Qualifications for the Senior Medical Social Worker:
  • Education: Master’s Degree in Social Work (MSW) required.
  • Licensure: New York State Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) required.
  • Experience: 2 years working with homeless populations and individuals with chronic health, mental health, or substance abuse conditions, including hospital discharge planning.
  • Skills: Strong interpersonal, organizational, and documentation skills, with proficiency in office software (e.g., email, word processing, spreadsheets, presentations).
  • Attributes: Compassionate, patient-focused approach with the ability to collaborate effectively in a dynamic environment.
  • Preferred: Bilingual in Spanish with knowledge of treatment practices and assessment tools.
