Job Overview - Respite Social Worker (LMSW):
Make a difference in New York, NY, by joining our team as a Respite Social Worker. You'll conduct psychiatric assessments, create person-centered treatment plans, and offer essential supportive services using evidence-based methods.

Location: New York, NY
Compensation: $37/hour
Schedule: 9:00 AM to 5:00 PM, Sunday to Thursday


Responsibilities of the Respite Social Worker Include:
  • Client Assessment and Planning: Assess clients’ substance abuse, health, mental health, and other care needs; recommend internal and external referrals.
  • Counseling and Psychoeducation: Provide individual and group counseling, prepare psycho-socials, and facilitate psycho-education groups.
  • Case Management: Deliver comprehensive case management services, assist clients in improving self-care, and achieving housing stability.
  • Discharge Planning: Manage discharge planning and engage and support clients 90 days post-discharge.
  • Documentation and Reporting: Manage client documentation, provide program narrative and/or statistical reports, and participate in ongoing QI and program evaluation activities.
Qualifications of the Respite Social Worker Include:
  • Licensure: New York State Licensed Masters in Social Work (LMSW) or Licensed Clinical Social Worker (LCSW) required.
  • Experience: 2 years of experience with homeless population and individuals with chronic health and/or mental health conditions and/or substance abuse disorders.
  • Skills: Ability to learn and provide evidence-based treatment practices, knowledge of treatment interventions and wellness management practices, proficiency in administering assessment tools, and delivering individual, group, and family counseling services.
  • Proficiency: Strong interpersonal skills, ability to create accurate and descriptive reports, and proficiency with office and internet software including email, word processing, spreadsheets, and presentations.
  • Language: Bilingual in Spanish preferred.
