Job Overview - Registered Nurse Evaluator (Home Health):
Join our dedicated client's team as a Registered Nurse Evaluator (Home Health). This full-time role involves conducting strength-based assessments for families with children (ages 0-21) who have behavioral health, psychiatric, or medical needs. The Registered Nurse Evaluator will work both telehealth and in-person appointments, supporting the development of Person-Centered Plans of Care (POC) to facilitate access to Home and Community Based Services (HCBS).
Location: Mid-Hudson Area (Multiple Locations)
Compensation: $50/hour
Schedule: 9:00 AM to 5:00 PM, Monday to Friday
Responsibilities as the Registered Nurse Evaluator (Home Health):
Assessment and Evaluation: Conduct comprehensive evaluations of children's health status, strengths, and care needs.
Plan of Care Development: Collaborate with families to create individualized HCBS Plans of Care.
Documentation: Utilize the State CANS assessment tool and MAXIMUS systems for accurate documentation.
Communication: Discuss healthcare options and services with children and families.
Collaboration: Coordinate with Family Support Coordinators and interdisciplinary team members.
Quality Review: Collect quality review data and maintain detailed case notes.
Resource Knowledge: Stay informed about community resources and network services.
Professional Development: Engage in ongoing education and certification in the CANS-NY assessment tool.
Qualifications for the Registered Nurse Evaluator (Home Health):
Experience: Minimum of 2 years of relevant experience as an RN.
Preferred: Experience in homecare, psychiatric, behavioral health, pediatrics, adolescent, foster care settings, group homes, school nursing, or care management preferred.
Assessment Experience: Experience using the UAS-CANS-NY system or similar systems like OASIS.
Technological Proficiency: Experience using computer systems for assessments and word processing tasks.
Communication Skills: Strong verbal and written communication skills.
