Job Overview: Registered Nurse Assessor
We are seeking a Registered Nurse Assessor for a hybrid role conducting home assessments and telehealth evaluations. This position requires a blend of in-field and remote work to assess and support patient needs.
Location: Queens, NY
Compensation: $52/hour
Schedule: Monday to Friday, (3 days dedicated to home assessments and 2 days to telehealth evaluations
Responsibilities as the Registered Nurse Assessor:
Conduct initial in-home pre-evaluations and develop personalized care plans.
Review and document consumer medical information.
Discuss health care options with consumers.
Communicate with Call Center for managed care enrollment.
Collaborate with care team members to improve service delivery.
Collect quality review data and maintain accurate records.
Identify opportunities for health promotion and illness prevention.
Perform other duties as assigned by management.
Qualifications for the Registered Nurse Assessor:
2+ years of recent experience in a home setting (e.g., CHHA, LHCSA, MLTC).
Experience with assessments similar to UAS.
Strong organizational, interpersonal, and communication skills.
Ability to work independently and manage multiple tasks.
Preferred: Experience with UAS-NY assessments and geriatric care.
Proficiency with technology and electronic health records.
