Job Overview – Registered Nurse Assessor:
We are seeking a Registered Nurse Assessor to join our trusted healthcare client's team in Brooklyn, NY! This hybrid role involves conducting home assessments and telehealth evaluations. This position requires a blend of in-field and remote work to assess and support patient needs.
Location: Brooklyn, NY
Compensation: $52/hour
Schedule: Monday to Friday, 3 days home assessments & 2 days telehealth
Responsibilities as the Registered Nurse Assessor:
Initial Evaluations: Perform initial in-home evaluations and develop care plans.
Medical Review: Review and document consumer medical information.
Health Discussions: Discuss health care options with consumers.
Communication: Communicate with the Call Center for managed care enrollment.
Collaboration: Collaborate with interdisciplinary care team members.
Quality Data Collection: Collect quality review data and maintain accurate records.
Health Promotion: Identify opportunities for health promotion and illness prevention.
Qualifications for the Registered Nurse Assessor:
Experience: 2+ years of recent experience in a home setting (CHHA, LHCSA, MLTC).
Assessment Skills: Experience with assessments similar to UAS.
Skills: Strong organizational, interpersonal, and communication skills.
Independence: Ability to work independently and manage multiple tasks.
Preferred: Experience with UAS-NY assessments and geriatric care.
Technical Proficiency: Proficiency with technology and electronic health records.
