Job Overview - Registered Nurse Assessor, Home Health:
Looking for a hybrid role in home healthcare? Join our client's expanding team in Central New York, as a Registered Nurse Assessor, Home Health. This temporary role involves conducting home and telehealth assessments for individuals with diverse health needs.

Location: Central New York (Multiple Locations)
Compensation: $52/hour
Schedule: 2 Days Home Assessments & 3 Days Telehealth (Hybrid)


Responsibilities as the Registered Nurse Assessor, Home Health:
  • Assessment and Evaluation: Perform initial in-home pre-evaluation of patients, assessing health status, strengths, care needs, and preferences to develop individualized long-term care service plans.
  • Documentation: Review consumer medical documentation, enter evaluation data into electronic forms, and document any concerns or conflicting information.
  • Care Coordination: Emphasize continuity of care, discuss healthcare options with consumers, and initiate communication for enrollment into managed care health plans.
  • Collaboration: Enhance communication with interdisciplinary care team members to improve care coordination and service delivery.
  • Quality Review: Collect quality review data, maintain detailed case notes, and support outcome measurements.
  • Health Promotion: Identify opportunities for health promotion and illness prevention.
  • Resource Knowledge: Maintain a comprehensive knowledge of community resources, payor requirements, and network services.
Qualifications for the Registered Nurse Assessor, Home Health:
  • Experience: Minimum 2 years in a home health setting within the past 10 years, including roles with CHHA, LHCSA, MLTC, or similar agencies.
  • Preferred: Recent experience conducting UAS-NY assessments (within the last year).
  • Skills: Proficient in health status evaluations and UAS criteria, strong understanding of nursing principles, state and agency regulations, excellent organizational and communication abilities, and effective in a fast-paced, deadline-driven environment.
