Job Overview: Registered Nurse - Care Delivery Manager
We are seeking a dedicated Registered Nurse Care Delivery Manager to support nursing standards of excellence and deliver high-quality care. This role involves overseeing daily operations, providing guidance, and ensuring organizational goals are met.
Location: Ridgewood, NY
Compensation: $115,000 - $118,000/year
Schedule: 8:30 AM - 5:00 PM, Monday to Friday
Responsibilities as the Registered Nurse - Care Delivery Manager:
Develop and execute personalized care plans.
Serve as an on-call manager during non-operational hours.
Run Clinical Delivery reports.
Balance staffing resources and ensure team coverage.
Coordinate care aligned with participant health goals.
Review and ensure accurate assessments.
Investigate and document incidents.
Ensure complete and accurate participant documentation.
Manage transitions in care settings.
Stay updated on health issues and clinical practices.
Recommend service and policy improvements.
Collaborate with HR for staff onboarding and training.
Evaluate performance and support direct reports.
Ensure regulatory compliance.
Resolve employee grievances.
Qualifications for the Registered Nurse Care Delivery Manager:
RN, BSN required or at least 5 years’ experience in healthcare.
Accredited Nursing Program completion.
Minimum 2 years of management experience in home health or similar settings.
Customer service experience required.
Experience with long-term care insurance is beneficial.
Minimum 1-year experience working with the elderly.
Supervisory experience required.
