Job Overview – Registered Nurse (RN) Community Health Program:
Empower communities with our client as a Registered Nurse (RN) in New York, NY, delivering holistic patient care, health education, and case management within a dynamic Community Health Program. This role is perfect for an experienced nurse with expertise in behavioral health, patient advocacy, and community-based care. Collaborate with interdisciplinary teams, assess patient needs, coordinate services, and support individuals in achieving better health outcomes.
Compensation: $92,000/year
Location: New York, NY
Schedule: Monday to Friday, 9:00 AM – 5:00 PM
Responsibilities as the Registered Nurse (RN):
Patient Assessment & Care Planning: Conduct health assessments, develop care plans, and oversee treatment.
Care Coordination: Collaborate with healthcare teams to implement and evaluate individualized care plans.
Health Education & Advocacy: Educate patients on disease prevention, medication management, and lifestyle changes.
Crisis & Emergency Response: Participate in crisis interventions and provide immediate medical care.
Community Outreach: Connect patients to primary care, housing services, and support programs.
Medication Management: Administer medications, monitor side effects, and ensure adherence to treatment plans.
Qualifications for the Registered Nurse (RN):
Licensure: Active RN license and current registration in New York State.
Experience: Minimum 1 year of post-licensure clinical nursing experience, preferably in behavioral health or community health settings.
Technical Skills: Strong knowledge of nursing principles, disease prevention, emergency interventions, and medication management.
Skills & Attributes: Ability to assess patient conditions, develop care plans, and collaborate across teams with cultural competence and sensitivity.
Other Requirements: Willingness to travel throughout New York City as needed.
