Job Overview - Registered Nurse (FACT):
Are you a compassionate Registered Nurse looking to make a difference in the lives of individuals with severe mental illness and substance use disorders? Join our Brooklyn, NY team as a Registered Nurse in our Forensic Assertive Community Treatment (FACT) program. This role focuses on providing comprehensive, community-based care to promote recovery and wellness.

Location: Brooklyn, NY
Compensation: $55/hour
Schedule: 9:00 AM to 5:00 PM, Monday to Friday


Responsibilities of the Registered Nurse (FACT) Include:
  • Community-Based Services: Provide 80% of medication management and health services in the community, including medication administration, side effect monitoring, phlebotomy, and health care coordination.
  • Documentation and Compliance: Complete and sign progress notes, nursing assessments, incident reports, and other required documentation following OMH regulations, DOHMH guidelines, and Medicaid billing standards.
  • Care Coordination: Coordinate medication management needs and health care services with the FACT prescriber and maintain medication administration schedules.
  • Interdisciplinary Collaboration: Coordinate care with family and community service providers, including hospitals, housing providers, shelter staff, criminal justice personnel, AOT, and primary health care providers.
  • Team Participation: Participate in daily team meetings, clinical meetings, and monthly nurses meetings to collaborate on clinical issues and client needs.
  • Evidence-Based Practices: Use motivational interviewing and trauma-informed approaches when delivering services.
Qualifications of the Registered Nurse (FACT) Include:
  • Licensure: Registered Nurse (RN) with current New York State license.
  • Experience: At least three years of experience working with adults in psychiatric nursing, substance use treatment settings, or social service settings for individuals experiencing homelessness or justice involvement.
  • Knowledge: Extensive knowledge of community treatment, support services, and resources.
  • Skills: Highly organized, self-motivated, and proficient in computers.
  • Language: Fluency in Spanish preferred.
