Job Overview – Registered Nurse (ACT Team):
Step into a pivotal role with our client as a Registered Nurse (ACT Team) in Manhattan, NY. You will be a core member of the Assertive Community Treatment (ACT) Team, delivering critical health assessment, counseling, and medication management services to promote recovery and wellness. This field-based role requires 80% of services to be provided in the community, ensuring optimal health outcomes for program participants.

Compensation: $92,000/year
Location: Manhattan, NY
Schedule: Monday to Friday, 9:00 AM – 5:00 PM (Full-Time)


Responsibilities as the Registered Nurse (ACT Team):
  • Health Assessments: Develop, implement, and evaluate individualized health assessments and nursing regimens for program participants.
  • Care Coordination: Maintain nursing standards while collaborating with mental health, substance abuse, housing, and medical care providers.
  • Medication Management: Oversee medication inventory, administer medications (including IMs and vaccines), and ensure compliance with protocols.
  • Education and Advocacy: Facilitate wellness sessions, educate ACT staff, and advocate for participants to secure medical care and community support.
  • Participant Oversight: Ensure participants receive annual physicals, dental care, and coordinated services with primary care providers and psychiatrists.
  • Documentation and Monitoring: Maintain accurate records, monitor vital signs, and provide crisis intervention as needed.
Qualifications for the Registered Nurse (ACT Team):
  • Education: Bachelor’s degree in Nursing (BSN) preferred.
  • Licensure: Registered Nurse (RN) with a current and valid New York State license.
  • Experience: At least 3 years in human services, specializing in substance use disorders, mental health, or criminal justice, with knowledge of related treatment models.
  • Skills: Strong organizational and self-motivation skills, along with excellent communication and interpersonal abilities.
  • Technical Skills: Proficiency in computer systems and electronic medical records.
  • Preferred: Fluency in Spanish and knowledge of community treatment resources and support services are strongly preferred.
