Role Overview:
We are seeking a Psychiatric Nurse to provide community-based support. In this position, you will conduct home visits, collaborate with a multidisciplinary team, and ensure individualized care to help participants maintain housing and reach recovery goals.
Schedule:
Monday - Friday 9 am - 5 pm
Compensation: $52.00 / HR Location: All of Philadelphia County

Responsibilities:
Coordinate behavioral and physical health needs
Monitor medications and provide nursing
Participate in Assertive Community Treatment team
Majority of work (75%) in the community
Complete health and biopsychosocial assessments
Consult on care plans and coordinate appointments
Liaise with community agencies and families
Maintain medical history and treatment records
Ensure timely documentation
Provide patient education and crisis intervention
Qualifications:
Licensed Registered Nurse in Pennsylvania
Experience with community mental health
Knowledge of psychiatric and clinical practices
Understanding of ACT model
Communication skills
Strong organizational and analytical skills
Valid driver's license
Verbal de-escalation abilities
Emotional stability
Willingness to be on-call
