Role Overview:
Working in collaboration with the Program team, the Registered Nurse (RN) plays a crucial role in conducting comprehensive mental and physical health assessments. The RN is responsible for planning, developing, coordinating, and providing treatment and rehabilitation, as well as professional support. This includes making referrals to community resources, such as community physicians, and facilitating consultations with other health professionals.
Position Type: 6 Month Temp Period Compensation:$60.00 - $65.00 / HR Location: Manhattan, New York Schedule: Monday - Friday, 9 am- 5 pm

Responsibilities:
Conduct psychiatric and physical assessments
Make appropriate referrals and advocate for clients
Provide medication and health management
Deliver treatment, rehab and support services
Develop, maintain and revise care plans
Engage new clients into care through outreach
Collaborate with other providers
Communicate with team and join community visits
Coordinate with community mental health services
Maintain timely and accurate records
Qualifications:
NY Registered Nurse License
Bachelor's Degree in Nursing
Experience with chronic mental illness and substance abuse
Medication management knowledge
Organized and efficient
Strong communication skills
Compassionate and culturally competent
Able to collaborate across systems
Familiarity with mental health regulations
Experience on multidisciplinary mobile team
