Role Overview:
A home care organization is seeking an experienced Certified Home Health Agency Registered Nurse to conduct skilled assessments and provide compassionate care to patients in their residences across Manhattan. The ideal candidate will develop nursing care plans, educate clients on self-management, and collaborate with multidisciplinary care teams to foster positive health outcomes.
Compensation: $105,000.00 - $110,000.00 Location: Manhattan, New York Schedule: Monday - Friday from 9 am to 5 pm
Responsibilities:
•   Performing initial and ongoing home evaluations
•   Providing direct clinical care during visits
•   Educating patients and caregivers
•   Coordinating care services and paraprofessionals
•   Documenting comprehensive notes
•   Traveling independently throughout region
Qualifications:
•   Active NY RN license and BSN degree
•   2+ years home health experience
•   Strong communication and assessment abilities
•   Comfortable learning EMR system
•   Proficiency with Microsoft Office
•   Reliable transportation
