Role Overview:
In the role of MLTC-Intake RN, you will play a crucial role in assessing potential new members for program eligibility and effectively explaining the program to them and their families.
Compensation: $95,000.00 - $110,000.00 Location: Brooklyn, New York - 4 days remote / 1 day in office Schedule: Monday - Friday, 9 am - 5 pm

Responsibilities:
Know required outreach activities
Assess eligibility and safety of potential members
Collaborate with intake/outreach team
Collaborate with clinical team on services
Communicate with potential members
Communicate with families and physicians
Ensure smooth transitions to managed care
Build relationships with referral sources
Stay updated on regulatory changes
Identify community resources
Qualifications:
Current NY Registered Nurse license
Bachelor’s degree in Nursing
1 year acute care experience
UAS-NY experience preferred
Home care management experience preferred
Proficient in MS Office
Communication and interpersonal skills
Bilingual in Spanish, Chinese etc. preferred
Compassion and cultural competency
Attention to detail
