Role Overview:
Join our client’s Forensic Assertive Community Treatment (FACT) team as a Registered Nurse. In this role, you will provide comprehensive medication management, health assessment, counseling, and psychoeducation to individuals (age 18 and up) diagnosed with severe mental illness (SMI) and co-occurring substance use disorders, who have histories of criminal legal system involvement.
Compensation: Up to $55.00 / HR
Location: New York City
Schedule: Monday to Friday, 9 AM - 5 PM
Responsibilities:
• Providing at least 80% of medication management and health services to consumers in the community, including medication administration (oral and intramuscular), monitoring side effects and adherence, phlebotomy, coordination of health care services, psychoeducation, mental status assessments, and rotating on team's after-hour emergency line
• Completing and signing progress notes, nursing assessments, incident reports, and other required documentation in accordance with New York State Office of Mental Health (OMH) regulations, New York City Department of Health and Mental Hygiene (DOHMH) guidelines, and Medicaid billing standards and regulations
• Coordinating with FACT prescriber on consumers' medication management needs, health care services, and maintaining medication administration schedules
• Coordinating care with family and other community service providers or collaterals, including hospitals, housing providers, shelter staff, criminal justice personnel, Assisted Outpatient Treatment (AOT), and primary health care providers
• Participating in daily team meetings, clinical meetings, and monthly nurses meetings to collaborate and discuss clinical issues and clients' needs
• Using motivational interviewing, culturally and developmentally appropriate, recovery-oriented, and trauma-informed approaches when delivering services
• Performing any other duties as required by Team Leader and senior clinical leadership
Qualifications:
• Registered Nurse (RN) with current New York State license
• At least three years of experience working with adults (ages 18 and up) in one or more of the following areas: psychiatric nursing, substance use treatment setting, social service setting(s) caring for individuals experiencing homelessness, justice involvement, or limited social support
• Extensive knowledge of community treatment, support services, and resources
• Highly organized and self-motivated
• Proficient in computers
• Fluency in Spanish preferred
