Role Overview:
Join a home health agency as a Certified Home Health Agency (CHHA) Home Care RN conducting skilled nursing visits to patients in Manhattan.
Compensation: $105,000.00 - $110,000.00 Location: Field-based role located in Bronx Schedule: Monday – Friday, 9 am – 5 pm
Responsibilities:
• Conducting home visits to provide skilled assessments, education, and nursing care
• Coordinating skilled services within interdisciplinary teams
• Directing patient’s paraprofessional services
Qualifications:
• Current NYS RN license
• Strong communication and organizational skills
• Reliable transportation for home visits
• BSN degree preferred
• Prior CHHA experience preferred
• EMR experience
