Job Overview – RN Intake Nurse (MLTC):
Compensation: $95,000 – $110,000/year
Location: Brooklyn, NY
Schedule: Monday to Friday, 9:00 AM to 5:00 PM (Hybrid)


Our client is hiring an RN Intake Nurse in Brooklyn, NY (Hybrid) to support a Managed Long-Term Care (MLTC) program. This hybrid role combines field-based UAS-NY assessments with remote documentation and care planning. Responsibilities include eligibility evaluations, community outreach, and cross-functional coordination to ensure smooth member onboarding. Candidates with home care, MLTC, or care coordination experience are strongly preferred.

Responsibilities as the RN Intake Nurse (MLTC):
  • Clinical Assessment & Eligibility: Conduct UAS-NY assessments to determine clinical eligibility and safety for MLTC enrollment.
  • Community Outreach: Engage potential members through outreach and provide education on program services.
  • Care Coordination: Work with Intake, Outreach, and Clinical teams to ensure smooth onboarding and care transitions.
  • Stakeholder Communication: Communicate with members, families, physicians, and referral sources to support enrollment.
  • Documentation & Compliance: Complete timely, accurate documentation in line with DOH, HIPAA, and internal standards.
Qualifications for the RN Intake Nurse (MLTC):
  • Licensure: Active New York State Registered Nurse (RN) license required.
  • Education: Bachelor’s degree in Nursing (BSN) required.
  • Experience: Minimum of 1 year in an acute care setting required, with home care, care management, or MLTC experience strongly preferred.
  • Technical Skills: Proficiency in UAS-NY and Microsoft Office Suite required, with EMR experience preferred.
  • Skills & Attributes: Strong communication, critical thinking, and interpersonal skills, with the ability to work independently and in team settings within a hybrid environment.
  • Language Skills: Bilingual in Spanish or Chinese preferred.
