Job Overview – RN Clinical Manager (Integrated Care):
Step into a pivotal leadership role with our client as an RN Clinical Manager (Integrated Care) in New York, NY. This role focuses on overseeing care transitions, ensuring quality home health services, and leading interdisciplinary teams to optimize patient outcomes. You will play a critical role in coordinating care, supervising nursing staff, and implementing best practices to enhance patient well-being and reduce hospital readmissions.
Compensation: $135,000 – $140,000/year
Location: New York, NY
Schedule: Monday to Friday, 8:30 AM – 5:30 PM
Responsibilities as the RN Clinical Manager:
Care Transition Coordination: Collaborate with hospital discharge planners, physicians, and care teams to develop and implement safe, patient-centered transition plans.
Clinical Assessments & Home Visits: Conduct home visits to assess patient health, living conditions, and support systems, ensuring continuity of care.
Nursing Care & Health Monitoring: Provide skilled nursing services, including medication management, wound care, vital sign monitoring, and chronic disease education.
Supervision & Staff Development: Lead and mentor Nurses, PCAs, and HHAs, ensuring competency, compliance, and adherence to best practices.
Regulatory Compliance & Quality Improvement: Maintain accurate documentation, ensure compliance with PACE and LHCSA regulations, and drive quality improvements.
Patient & Family Education: Educate patients and families on care plans, medication adherence, and lifestyle modifications to promote effective self-management.
Qualifications for the RN Clinical Manager:
Education: Degree from an NLN-accredited nursing school required (BSN preferred).
Experience:Â 3+ years of clinical nursing experience, including 1-2 years in home care, care transitions, case management, or PACE programs (long-term care or managed care highly preferred).
Licensure: Active and unrestricted Registered Nurse (RN) license in New York State.
Skills & Attributes: Strong clinical assessment, critical thinking, and communication skills with the ability to manage multiple cases and lead interdisciplinary teams.
Technical Proficiency: Experience with electronic health record (EHR) systems and proficiency in documentation and administrative tasks.
Mobility & Travel: Ability to travel between patient homes, work in varied environmental conditions, and walk between locations as needed.
