Job Overview – RN Case Manager (I/DD):
Join our client’s Brooklyn, NY team as an RN Case Manager (I/DD), ensuring high-quality healthcare coordination and advocacy for individuals with intellectual and developmental disabilities. This full-time role is ideal for a compassionate RN with strong case management experience, responsible for health assessments, care planning, and medical oversight to enhance patient well-being and quality of life. This position includes fieldwork, with home visits in Brooklyn and Queens every 90 days to ensure continuity of care and patient support.

Compensation: $50/hour
Location: Brooklyn, NY (Includes fieldwork across Brooklyn and Queens)
Schedule: Monday to Friday (Full-Time)


Responsibilities as the RN Case Manager (I/DD):
  • Patient Assessments: Conduct quarterly and annual nursing evaluations with accurate documentation.
  • Care Coordination: Serve as a liaison between individuals, providers, and family care teams.
  • Medical Oversight: Ensure compliance with health regulations and coordinate referrals.
  • Medication Management: Oversee prescribing, administration, and documentation per NY regulations.
  • Education & Training: Train individuals and caregivers on health maintenance and medication management.
  • Hospital & Discharge Planning: Facilitate seamless care transitions during hospital admissions and discharges.
Qualifications for the RN Case Manager (I/DD):
  • Licensure: Active Registered Nurse (RN) license in New York State.
  • Experience: Background in developmental disabilities, home care, or skilled nursing facilities (SNF) preferred.
  • Technical Skills: Strong knowledge of case management, healthcare documentation, and regulatory compliance.
  • Skills & Attributes: Excellent communication, interpersonal, and organizational skills.
  • Other Requirements: Valid NYS driver’s license required.
