Job Overview - RN-Clinical Assessment Outreach Manager:
Our healthcare client is seeking a dedicated RN - Clinical Assessment Outreach Manager to join their team in New York. This full-time role involves educating referral sources and prospects about the Program of All-Inclusive Care for the Elderly (PACE), conducting clinical assessments, and guiding prospects through the onboarding process.

Location: New York, NY
Compensation: $115,000 - $120,000/year
Schedule: 8:30 AM to 5:30 PM, Monday to Friday

Responsibilities as the RN-Clinical Assessment Outreach Manager:
  • Referral and Enrollment: Achieve referral and enrollment goals by building relationships with referral partners and engaging prospective enrollees and their caregivers.
  • Verification and Compliance: Ensure high performance in pre-enrollment verifications for Medicare, Medicaid, and PACE eligibility while maintaining knowledge of CMS, DOH, and PACE guidelines, ensuring compliance with state and federal regulations.
  • Program Education and Event Coordination: Educate various audiences on PACE program benefits through lectures, presentations, events, and meetings; collaborate with the Events Supervisor to participate in community events to promote PACE.
  • Intake Assessments: Assist with qualification screening, documentation collection, benefit education, site tours, home visits, follow-up, and conduct pre-enrollment assessments to determine eligibility and care needs.
  • Documentation and Planning: Record all interactions and outcomes in the clinical management/electronic record system and develop 30-day service plans with participants and their families/caregivers.
  • Administrative Support: Prepare informational materials, track activities, compile statistical data, and work with Compliance and Marketing to develop outreach and intake materials.
  • Process Improvement and Staff Management: Advise on process improvements, review performance metrics, manage staff recruitment, training, and supervision, ensuring efficient team transitions.
  • Patient Care: Ensure all activities comply with practice scope and standardized competencies, maintaining effective communication with stakeholders.

Qualifications for the RN-Clinical Assessment Outreach Manager:
  • Education: BS in Nursing or health-related field preferred.
  • Experience: Minimum of 1 year with frail or elderly populations; 3-5 years in geriatrics, home care, MLTC, MAP, D-SNP, and/or PACE.
  • License: Active and unrestricted New York State RN license.
  • Knowledge: Understanding of community-based long-term care services for dual-eligible and frail adult populations, and federal/state regulations.
  • Skills: Strong interpersonal, organizational, and communication skills; ability to handle urgent situations and meet targets.
  • Technical Proficiency: Proficiency in Microsoft Word, Excel, and statistical analysis; experience with Care Compass preferred.
  • Language: Bilingual preferred.
