Job Overview - RN, Nurse Assessor (Home Health):
Join our healthcare client's dedicated team as an RN, Nurse Assessor (Home Health). This hybrid role involves conducting home and telehealth assessments for individuals with diverse health needs, developing comprehensive care plans, and ensuring continuity of care through effective coordination and communication.

Location: East Hudson, NY
Compensation: $52/hour
Schedule: 2 Days Home Assessments & 3 Days Telehealth (Hybrid)

Responsibilities as the RN, Nurse Assessor (Home Health):
  • Assessment and Evaluation: Perform initial in-home pre-evaluation of patients, assessing health status, strengths, care needs, and preferences to develop individualized long-term care service plans.
  • Documentation: Review consumer medical documentation, enter evaluation data into electronic forms, and document any concerns or conflicting information.
  • Care Coordination: Emphasize continuity of care, discuss healthcare options with consumers, and initiate communication for enrollment into managed care health plans.
  • Collaboration: Enhance communication with interdisciplinary care team members to improve care coordination and service delivery.
  • Quality Review: Collect quality review data, maintain detailed case notes, and support outcome measurements.
  • Health Promotion: Identify opportunities for health promotion and illness prevention.
  • Resource Knowledge: Maintain a comprehensive knowledge of community resources, payor requirements, and network services.
  • Professional Development: Engage in ongoing education and certification in the CANS-NY assessment tool.
Qualifications for the RN, Nurse Assessor (Home Health):
  • Experience: Minimum 2 years in a home health setting within the past 10 years, including roles with CHHA, LHCSA, MLTC, or similar agencies.
  • Preferred: Recent experience conducting UAS-NY assessments (within the last year).
  • Skills: Proficient in health status evaluations and UAS criteria, strong understanding of nursing principles, state and agency regulations, excellent organizational and communication abilities, and effective in a fast-paced, deadline-driven environment.
