Job Overview - RN, Home Care (CHHA):
Do you have a passion for patient care and are eager to work in a supportive, multidisciplinary environment? Our client is seeking a dedicated Home Care Registered Nurse to join their expanding healthcare team. This role involves conducting home visits to provide skilled clinical assessments, patient education, and direct nursing care to patients in their homes.
Location: Bronx, NY
Compensation: $105,000 - $110,000/year
Schedule: 9:00 AM to 5:00 PM, Monday to Friday
Responsibilities as the RN, Home Care (CHHA):
Home Visits: Conduct home visits to provide skilled clinical assessments, patient education, and direct nursing care.
Care Coordination: Coordinate skilled services within the patient’s interdisciplinary team and direct the patient’s paraprofessional services.
Patient Education: Educate patients and their families on conditions, treatment goals, and home care practices.
Documentation: Maintain accurate and up-to-date patient records, including treatment notes and progress reports.
Team Collaboration: Communicate and collaborate with other healthcare professionals for a multidisciplinary approach.
Qualifications for the RN, Home Care (CHHA):
License and Education: Current NYS RN license and BSN degree required.
Experience: Prior CHHA experience preferred; working knowledge of Home Care Home Base EMR is preferable.
Skills: Strong organizational, communication, written, and customer service skills.
Technical Proficiency: Competent with Microsoft Office.
Transportation: Must have a reliable mode of transportation for home visits.
