Job Overview - RN- Psychiatric (Fieldwork):
Our trusted healthcare client is seeking a dedicated Full-time Psychiatric Nurse, specializing in Fieldwork, to join their team in Philadelphia, PA. This role involves providing community-based supports to individuals, ensuring compliance with agency service contracts, policies, and procedures.

Location: Philadelphia, PA
Compensation: $52/hour
Schedule: 9:00 AM to 5:00 PM, Monday to Friday


Responsibilities as the RN- Psychiatric (Fieldwork):
  • Care Coordination: Coordinate the behavioral and physical health care needs of service participants.
  • Medication Monitoring: Monitor participant’s medication and provide psychiatric nursing services consistent with physician orders and standard nursing practices.
  • Community Outreach: Conduct approximately 75% of work in the community, including home-based visitations, to ensure participants receive individualized services.
  • Team Collaboration: Participate as a member of a focused team providing Assertive Community Treatment services, helping participants reach their maximum level of functioning in the community.
  • Assessments: Complete health and biopsychosocial assessments for every active participant.
  • Consultation: Consult with team members regarding the development and implementation of health care needs and goals, coordinating routine medical, vision, lab, and dental appointments.
  • Record Maintenance: Maintain a record of medical history and ongoing medical and treatment records, ensuring service documentation is submitted timely.

Qualifications for the RN- Psychiatric (Fieldwork):
  • Licensure: Board Certified Licensed Registered Nurse in the State of Pennsylvania.
  • Experience: Prior experience in servicing community mental health facilities and their psychiatric needs.
  • Knowledge: Comprehensive knowledge of psychiatric, clinical, and psychosocial rehabilitation practices.
  • Service Model Understanding: Understanding of the Assertive Community Treatment (ACT) model of service.
  • Skills: Strong organizational, analytical, and communication skills for effective interaction with all levels of the behavioral health system.
  • Driver’s License: Valid driver’s license with an acceptable driving record required.
