Job Description

Job Overview – Physical Therapist:
Step into a rewarding role as a Physical Therapist with our client in Brooklyn, NY. In this full-time position, you will deliver therapeutic care to patients, restoring function, relieving pain, and preventing disability. Working in collaboration with interdisciplinary teams, you will support patients in both clinical and community settings, enhancing their quality of life through personalized treatment plans.

Compensation: $60/hour
Location: Brooklyn, NY
Schedule: Monday to Friday, 9:00 am – 5:00 pm


Responsibilities as the Physical Therapist:
  • Patient Evaluation: Perform comprehensive assessments and deliver therapy at PACE sites, homes, or nursing facilities.
  • Care Planning: Collaborate with interdisciplinary teams to create and implement individualized care plans.
  • Documentation: Maintain accurate records, including assessments, progress notes, and treatment plans.
  • Discharge Coordination: Manage equipment needs and ensure smooth patient transitions.
  • Education: Provide clinical instruction to staff and students and participate in training programs.
  • Patient Safety: Ensure a safe environment, adhering to safety protocols and infection control standards.
Qualifications for the Physical Therapist:
  • Education: Successful completion of a Physical Therapy program from an APTA-accredited university.
  • Licensure: Current New York State licensure as a Registered Physical Therapist.
  • Experience: Minimum of 1 year of experience as a Physical Therapist with geriatric or frail populations.
  • Skills: Ability to establish rapport with patients and staff, strong documentation skills, and proficiency in Care Compass and other EMRs.
  • Attributes: Knowledge of PACE regulations, a commitment to patient-centered care, and the ability to work collaboratively in a team environment.
  • Travel: Ability to travel to patient homes, nursing facilities, and other sites as required.
  • Language Skills: Bilingual candidates are strongly preferred.
