Job Overview – Physical Therapist:
Step into a meaningful role with our client as a Physical Therapist in Brooklyn, NY. In this position, you will provide comprehensive physical therapy services to restore function, relieve pain, and prevent disability for participants referred through a medically guided program. You will work closely with an interdisciplinary team to deliver individualized care plans and improve the quality of life for participants.
Compensation: $99,000 - $113,000/year
Location: Brooklyn, NY
Schedule: Monday to Friday, 9:00 AM – 5:00 PM
Responsibilities as the Physical Therapist:
Assessments and Evaluations: Conduct thorough assessments and treatment sessions at the PACE site or in the field.
Treatment Planning: Develop individualized care plans in collaboration with the interdisciplinary team, ensuring regulatory compliance.
Documentation: Maintain accurate documentation of assessments, progress notes, and care plans.
Clinical Expertise: Provide guidance on therapeutic exercises, mobility, and equipment use, and consult with other professionals as needed.
Safety and Environment: Ensure a clean, safe environment and adhere to safety protocols for participant well-being.
Discharge Planning: Initiate timely discharge plans and coordinate necessary equipment and support services.
Qualifications for the Physical Therapist:
Education: Graduation from a Physical Therapy program accredited by the APTA required.
Licensure: Valid New York State licensure as a Registered Physical Therapist is required.
Experience: Minimum of one (1) year of experience as a Physical Therapist with geriatric or frail populations.
Skills: Ability to establish rapport with participants and staff, with strong interpersonal and communication skills.
Technical Proficiency: Familiarity with PACE regulations and electronic medical records (e.g., Care Compass) is preferred.
Additional: Ability to travel to participants’ homes, nursing facilities, and clinic sites.
Preferred: Bilingual candidates are strongly preferred.
