Job Overview - Peer Specialist - Case Manager:
We are seeking a Peer Specialist, also known as a Case Manager, to join the our client's team based in Manhattan, New York. This role focuses on providing peer support and case management services to individuals with frequent interactions with mental health, substance abuse, criminal justice, and homeless services systems.
Compensation: $37,688 - $45,000 per year
Location: New York, NY
Schedule: Monday to Friday, 9:00 AM to 5:00 PM
Responsibilities as the Peer Specialist - Case Manager:
1. Peer Specialist Services:
Offer peer support and counseling to individuals with mental health diagnoses, trauma, or substance use.
Utilize personal recovery experience to guide and mentor others.
Facilitate peer-led activities and provide educational sessions on various recovery topics.
2. Case Management:
Develop and implement personal recovery plans including wellness recovery, safety, and crisis intervention plans.
Coordinate with various service systems to advocate and liaise for participants.
Maintain accurate and timely client case records.
Qualifications for the Peer Specialist - Case Manager:
High School Diploma/GED required.
Proven ability to work with individuals in recovery from mental illness, utilizing lived experiences beneficially.
Strong interpersonal skills, ability to communicate effectively in a trauma-informed and culturally competent manner, and capacity to engage with various community services.
Must be willing to travel locally.
