Position Overview:
A Health Homes program in Staten Island is seeking a Patient Navigator to assist and follow up with clients. The role involves chart reviews, clerical duties, appointment scheduling, client transportation, home visits, linking clients to services, and supporting operations.
Compensation: $24.00/HR Location: Staten Island, New York Schedule: Monday - Friday, 9 AM - 5 PM
Responsibilities:
• Review client charts and monitor service plans
• Perform clerical duties to maintain records
• Schedule appointments and transport clients
• Conduct home visits to follow up with clients
• Provide referrals to medical and social services
• Document all client encounters
Qualifications:
• High school diploma or equivalent
• Experience with underserved groups preferred
• Bilingual English/Spanish a plus
• Valid NY driver’s license
• Exceptional communication skills
