Job Overview - Patient Care Nurse (RN):
Join our healthcare client in New York City, as a Patient Care Nurse (RN) overseeing home-based healthcare services. In this full-time role, you will manage patient care plans, conduct assessments, and supervise home visits to ensure high-quality care.
Compensation: $125,000 - $150,000/year
Location: New York, NY
Schedule: 8:30 AM to 5:30 PM, Monday to Friday
Responsibilities as the Patient Care Nurse (RN):
Care Planning: Perform initial nursing assessments and develop/update personalized care plans every 6 months or as needed.
Medication Management: Ensure accurate medication lists, assist with pre-pouring medications, and manage medication administration for clients.
Quality Assurance: Conduct home visits every 3-4 months to supervise service quality and provide oversight for specialized services and sick clients.
Supervision & Training: Supervise and train CAREGivers, including in-office and field training, ensuring compliance with delegated specialized services.
Documentation: Maintain detailed records of all client interactions, medication administration, and specialized services both electronically and physically.
Qualifications for the Patient Care Nurse (RN):
Licensing: Current, unrestricted Registered Nursing License in New York State.
Experience: 5-10 years of Nursing experience, including Delegation and Employee Management.
Skills: Expertise in supervising compliance and delegating nursing tasks per NY State Department of Health guidelines.
Travel: Ability to commute using New York City public transportation to perform at least 5 client visits daily.
