Role Overview:
Join our client as a Patient Care Coordinator, where you will utilize your clinical and administrative skills to ensure high-quality patient care. The ideal candidate is a Registered Nurse with at least two years of pediatric experience.
Compensation: $100,000.00
Location: Long Island City, New York
Schedule: Monday to Friday, 8 am - 4p m or 9 am-5 pm
Responsibilities:
• Anticipating patient needs and ensuring completion of pre-visit planning
• Reviewing daily patient appointments and preparing, distributing, and analyzing daily patient rosters
• Coordinating and documenting daily patient care conferences
• Providing proactive outreach to high-risk patients and facilitating advanced management of their care
• Developing individualized care plans by collaborating with providers and assisting patients in setting treatment goals
• Educating patients on their care plans and properly documenting interactions in eCW
• Participating in QI Team meetings and collaborating with Benefits Counselors, Outreach staff, and the HIM supervisor to ensure appropriate patient scheduling, flow, and documentation procedures
• Performing other duties, assignments, or special projects as needed
Qualifications:
• Valid New York State RN License
• Minimum of 2 years of pediatric experience
• Prior clinical experience preferred; administrative experience also preferred
