Job Overview - Outreach Specialist:
Do you have a knack for connecting with people and a passion for community outreach? Join our dedicated team as an Outreach Specialist in Staten Island, NY, where you'll play a crucial role in identifying and enrolling new clients for our Health Homes program.
Location: Staten Island, NY
Compensation: $22/hour
Schedule: Monday to Friday, 9:00 a.m. - 5:00 p.m.
Responsibilities as the Outreach Specialist:
Identify clients for outreach in coordination with the Outreach Supervisor.
Conduct home visits, phone calls, and mail letters to enroll eligible clients.
Document progress notes and screening tools for clients.
Drive agency vehicle to provide outreach services.
Maintain accurate documentation in the program's database.
Qualifications for the Outreach Specialist:
High School Diploma or equivalent.
Experience with the homeless or HIV population preferred.
Bilingual in Spanish/English is a plus.
Clean, valid driver's license with the ability to drive a passenger van required.
Exceptional communication skills, both oral and written.
