Role Overview:
Join a healthcare organization as a Nurse Practitioner Supervisor (NPS), collaborating with community primary care physicians to coordinate care for participants in the Program of All-Inclusive Care for the Elderly (PACE).
Compensation: $90.00/HR (1099) or $75.00 - $80.00/HR (W2)
Location: Bronx, New York
Schedule: Monday - Friday, 8:30 am - 5:00 pm (1-hour unpaid lunch)
Responsibilities:
• Overseeing care coordination for assigned PACE site participants
• Conducting annual wellness assessments and managing acute care needs
• Reviewing medical histories and documenting diagnoses
• Participating in clinical documentation improvement initiatives
• Co-managing high-risk patients with PCPs
• Attending interdisciplinary care team meetings
• Supervising home care services and care plan delivery
• Providing on-call and emergency coverage as needed
Qualifications:
• Nurse Practitioner graduate from NYSED-approved program
• Licensed NP in New York State
• 2+ years of clinical experience in geriatrics, primary care, or internal medicine
• DEA certification and AANP/ANCC board certification
• Strong communication, analytical, and organizational skills
• Proficiency with Microsoft Office
• Bilingual abilities preferred
