Role Overview:
The nurse practitioner will be responsible for providing primary medical care and care management/coordination to a panel of assigned members residing in our client's contracted nursing facility.
Compensation: $120,000.00 - $140,000.00
Location: Bronx, New York
Responsibilities:
• Provide primary medical care and care coordination to assigned star members in a contracted nursing facility
• Collaborate with the member's PCP and the nursing home Medical Director
• Perform regular and acute assessments following the SNF's Medical Staff by-laws and Federal and State Health Codes
• Work closely with the nursing home's Interdisciplinary Care Team (ICT)
Qualifications:
• Ideal candidate must possess a graduate degree from a Nurse Practitioner program
• Must have a minimum of two (2) years full-time experience as a Nurse Practitioner, preferably in clinical geriatric care
• Ability to work independently, develop a differential diagnosis and initiate treatment
• Board Certification from AANP or ANCC
• Current DEA Certificate, BCLS
