Role Overview:
We are seeking a Nurse Practitioner to coordinate and oversee care for participants in our client’s elder services program. You will conduct assessments, create integrated care plans, manage complex cases, enable care transitions, and help remove barriers to whole-person wellness.
Compensation: $90.00 (1099) or $75.00 - $80.00 (W2)
Location: Queens, New York
Schedule: Monday - Friday, 8:30 am - 5:00 pm (1 Hour Unpaid Lunch)
Responsibilities:
• Performing wellness assessments and primary care
• Representing the Medical Director as needed
• Co-managing high-risk patients with physicians
• Communicating across interdisciplinary teams
• Educating on health conditions and self-management
• Ensuring accurate coding/documentation and follow-up
• Conducting home visits as necessary
• Providing urgent care and emergency coverage
Qualifications:
• MSN or DNP Degree
• Licensed NP in New York
• 2+ years geriatrics experience
• Analytical and healthcare operations talents
