Role Overview:
We are seeking a full-time Nurse Practitioner to provide coordinated care and optimize the well-being of seniors in our client’s innovative program. You will conduct assessments, create integrated plans, enable smooth transitions, and help participants thrive through patient-centered support.
Compensation: $90.00 (1099) or $75.00 - $80.00 (W2)
Location: Brooklyn, New York
Schedule: Monday - Friday, 8:30 am - 5:00 pm (1 Hour Unpaid Lunch)
Responsibilities:
• Performing wellness evaluations and managing acute needs
• Co-managing complex cases with physicians
• Communicating across interdisciplinary teams
• Educating on health conditions and self-care
• Ensuring accurate coding/documentation
• Making home visits as necessary
• Providing emergency/urgent care coverage
Qualifications:
• MSN or DNP Degree
• Licensed NP in New York
• 2+ years of geriatrics experience
• Analytical and healthcare operations abilities
