Job Overview - Nurse Evaluator (RN):
Step into a temporary role as a Nurse Evaluator (RN) with our client in Western New York, conducting evaluations for children, collaborating with families, and developing care plans. This flexible position offers opportunities across multiple locations, allowing you to work where it's most convenient.

Compensation: $50/hour
Location: Western New York (Multiple locations available, including Ithaca, Elmira, Corning, and surrounding areas)


Responsibilities as the Nurse Evaluator (RN):
  • Care Development: Collaborate with children and families to develop Home and Community-Based Services (HCBS) Plans of Care (POC) that address health status, strengths, and functional care needs.
  • Evaluations: Conduct thorough evaluations of children’s health status, including functional care needs and preferences, guiding individualized long-term care service plans.
  • Assessments: Utilize the State CANS assessment tool and case management systems to document health status and transmit required data.
  • Coordination: Work closely with interdisciplinary care teams to ensure continuity of care, reducing gaps and improving overall service delivery.
  • Planning: Discuss healthcare options, vendor choices, and waiver options with children and families to support informed decision-making.
Qualifications for the Nurse Evaluator (RN):
  • Education: Bachelor’s degree in Nursing (BSN) or an accredited RN program.
  • Experience: 2+ years as an RN in Home Care, Psychiatric/Behavioral Health, Pediatrics, or Care Management.
  • Assessments: Proficient in UAS-CANS-NY or similar systems (OASIS), with experience in conducting assessments.
  • Care Planning: Skilled in developing and managing care plans.
  • Technical Skills: Strong proficiency in computer systems for assessments and documentation.
  • Preferred Experience: Background in homecare, foster care, group homes, or school nursing is a plus!
