Job Overview – Nurse Evaluator (RN):
Join our client’s healthcare team as a Nurse Evaluator in the Capital District, NY! In this role, you will conduct strength-based assessments for families with children (ages 0-21) who have behavioral health, psychiatric, or medical needs. You will receive continuous support and education for professional development and annual certification in the CANS-NY assessment tool.
Location: Capital District, NY (Various Locations)
Compensation: $50/hour
Schedule: Flexible, includes telehealth and in-person appointments
Responsibilities as the Nurse Evaluator (RN):
Assessment and Evaluation: Conduct strength-based assessments for children with behavioral health and psychiatric issues.
Plan of Care Development: Collaborate with families to develop Home and Community-Based Services (HCBS) Plans of Care.
Documentation: Utilize the State CANS assessment tool to document health status, strengths, care needs, diagnoses, and preferences.
Care Coordination: Work with Family Support Coordinators to address the family's case management needs.
Communication: Enhance communication and collaboration with interdisciplinary care team members.
Quality Review: Collect quality review data and support outcome measurements.
Community Resources: Maintain comprehensive knowledge of community resources and services for the target population.
Qualifications for the Nurse Evaluator (RN):
Experience: 2+ years of relevant RN experience.
Specializations: Experience in homecare, psychiatric/behavioral health, pediatrics, adolescent care, foster care, or care management.
Assessment Skills: Proficient in conducting face-to-face, telephonic, or telehealth assessments.
Technological Proficiency: Consistent experience using computer systems for assessments and word processing tasks.
Plan Development: Experience in developing Plans of Care.
