Position Overview:
The Nurse Care Manager will coordinate whole-person care for members to maintain independent living through conducting in-home assessments, care planning, advocacy, and authorizing covered health services across care settings.
Compensation: $95,000.00 - $110,000.00 Location: Queens, New York Schedule: Flexible based on availability
Responsibilities:
• Work to enable members to remain in their preferred living situation
• Conduct comprehensive assessments and close monitoring
• Authorize and coordinate covered services across payers
• Collaborate with members, families, providers, and care teams
• Provide transition of care facilitation and care navigation
• Promote quality, cost-effective health outcomes
Qualifications:
• Current NYS RN license and registration
• Bachelor's degree in Nursing (BSN)
• 3+ years of RN experience
• Clinical expertise in geriatrics preferred
• Experience with long-term care preferred
• Willingness to travel to Long Island
