Nurse Care Manager | Fieldwork (1-day in office a week)
Position Overview:
Our client is seeking a bilingual per diem registered nurse to conduct home visits and assessments and coordinate care for Atlantic County, NJ patients. This role offers flexible scheduling, paid lunch, and work options. Home care experience is preferred.
Compensation: $52.00/HR Location: Atlantic City, New Jersey Schedule: Flexible based on availability
Responsibilities:
• Perform initial and ongoing patient assessments
• Develop and implement care plans
• Provide skilled nursing services in the home
• Coordinate care with providers and community services
• Consistently follow clinical guidelines and safety practices
• Maintain timely and accurate documentation
Qualifications:
• Active NJ RN license in good standing
• 1+ years of nursing experience (home care preferred)
• Bilingual (English/Spanish) communication skills
• Strong clinical judgement and assessment abilities
• Excellent professionalism, communication and teamwork
• Reliable transportation for home visits
