Job Overview – Mental Health Clinician (Licensed):
Step into a role with our client as a Mental Health Clinician in New York, NY, within the ACT program, delivering community-based care for individuals with severe mental illness, including Forensic ACT clients. Work with a multidisciplinary team to provide crisis intervention, skill-building, and support for client stability.
Compensation: $50,000 - $63,000/year
Location: New York, NY
Schedule: Monday to Friday, 9:00 AM to 5:00 PM
Responsibilities as the Mental Health Clinician (Licensed):
Care Coordination: Collaborate with a multidisciplinary team to provide ACT-model services, participating in daily meetings to align on client needs and progress.
Community Services: Ensure 80% of client interactions occur in community settings, including homes, workplaces, and social environments, to meet individual needs.
Assessments: Conduct needs assessments, psychosocial evaluations, and develop individualized service and crisis prevention plans, utilizing evidence-based practices such as CBT and trauma-informed care.
Documentation: Maintain accurate and timely documentation in electronic health records, meeting monthly service requirements and reporting obligations.
Crisis Management: Offer on-call crisis intervention on a rotating basis, assessing and addressing risks related to violence, substance use, trauma, and other factors.
Qualifications for the Mental Health Clinician (Licensed):
Licensure: LMSW, LMHC-LP, or LMHC required.
Experience: Minimum of 2 years working with adults experiencing severe mental illness (preferably in community or forensic settings).
Skills: Proficiency with electronic health records (experience with AWARDS by Footholds preferred).
Language: Spanish fluency strongly preferred.
Other Requirements: Eligibility for a New York State ID (driver’s or non-driver’s license).
