Job Overview – Mental Health Case Manager:
Compensation: $25/hour
Location: Bronx, NY
Schedule: Monday to Friday, 9:00 AM to 5:00 PM


Support vulnerable individuals in achieving independent living as a Mental Health Case Manager with our client in the Bronx, NY. In this community mental health role, you will conduct assessments, develop service plans, and provide direct case management through home visits, crisis intervention, and referrals to healthcare, housing, and support services. You’ll collaborate with providers, monitor client progress, and maintain documentation to ensure program compliance. Ideal for compassionate professionals with mental health case management experience in field-based settings.

Responsibilities as the Mental Health Case Manager:
  • Assessment & Service Planning: Conduct comprehensive assessments and develop individualized service plans with long- and short-term goals.
  • Direct Case Management: Maintain weekly client contact (including monthly home visits) and assist with medical care access, medication adherence, daily living skills, and housing retention.
  • Crisis Intervention & Support: Provide crisis intervention and ongoing support to promote client safety and stability.
  • Coordination & Advocacy: Coordinate referrals to medical, mental health, and social service providers and advocate for client needs, including education on budgeting and tenant responsibilities.
  • Monitoring & Documentation: Track client progress, maintain detailed case notes, and complete required documentation, schedules, and supervision.
Qualifications for the Mental Health Case Manager:
  • Education: Bachelor’s degree in Social Work, Public Administration, Psychology, or a related field required.
  • Experience: 1+ year of case management in mental health or community-based settings, preferably with clients with mental illness or substance use.
  • Technical Skills: Basic proficiency in Microsoft Office (Excel, Word) and electronic documentation systems.
  • Skills & Attributes: Strong communication and counseling skills, ability to manage multiple priorities, and a compassionate, culturally sensitive approach to field-based work with diverse populations.
  • Language Skills: Bilingual in Spanish and English strongly preferred.
